At Mayfield Smiles, we make every attempt to make things easy on our clients. For that reason, we accept Visa, MasterCard, and Discover as forms of payment. Because we deliver top-notch, family-oriented services at a reasonable cost, we kindly request for payment to be made at the time of service unless other arrangements have been made in advance. If you have any questions in regards to your account, please feel free to give us a call at 440-461-5482.

Please keep in mind that you are fully responsible for any and all fees charged by our office regardless of your insurance coverage.

We send monthly statements to our clients. However, most insurance companies respond within 4-6 weeks from the time of service. If for any reason your statement does not reflect insurance payment within the 4-6 weeks, please call our office at 440-461-5482. After insurance has paid, any remaining balance is your responsibility. We can make arrangements for monthly payment plans, however, this must be done prior to your procedure.

These are a few of the major insurances for which we are a Preferred Provider. Anthem Dental Blue 200/300 PPO Plus, Cigna, Connection Dental Network, Core Source, Delta Premier, Delta PPO, GEHA, Guardian, Humana PPO, SunLife & Health Insurance, United Concordia, and Principal. There are multiple smaller insurance companies that we also are a Preferred Provider for and accept. We cannot guarantee we are part of your plan. We take most PPO insurance plans offered by employers as well as people who do not have insurance.
